Please arrive 15 MINUTES prior to your appointment to Check-In

Please give our office a call at 408-249-5121 so we can check you in over the phone. ·

BOTH PARENT & PATIENT must wear a MASK into the office.

​

Only one Parent/Guardian is allowed to accompany the patients

​

We will take your temperature before entering the office.

​

You will notice our investments in additional PPE and equipment you may not be accustomed to

including face shields, extra masks, hair covers, gowns,  etc.

We have updated our policy for visits to insure that you and our team can be seen safely.
